{"id":1454,"date":"2020-04-23T14:47:40","date_gmt":"2020-04-23T13:47:40","guid":{"rendered":"http:\/\/blogs.bath.ac.uk\/iprblog\/?p=1454"},"modified":"2020-05-01T10:15:42","modified_gmt":"2020-05-01T09:15:42","slug":"older-informal-carers-where-is-their-badge-of-honour","status":"publish","type":"post","link":"https:\/\/blogs.bath.ac.uk\/iprblog\/2020\/04\/23\/older-informal-carers-where-is-their-badge-of-honour\/","title":{"rendered":"Older informal carers \u2013 where is their badge of honour?"},"content":{"rendered":"<p class=\"p1\"><span class=\"s1\"><a href=\"https:\/\/researchportal.bath.ac.uk\/en\/persons\/rachel-forrester-jones\"><i>Professor Rachel Forrester-Jones<\/i><\/a><\/span><span class=\"s2\"><i> is Director of the Centre for the Analysis of Social Policy (CASP) at the University of Bath.<\/i><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Local authorities (LAs) in England are stating that <a href=\"https:\/\/www.theguardian.com\/society\/2020\/apr\/17\/english-councils-set-for-1bn-bailout-coronavirus-ravages-finances\"><span class=\"s2\">coronavirus has ravaged finances <\/span><\/a>and are asking for a government bailout; the pandemic merely highlighting the result of consistent underfunding due to a decade of austerity measures. <a href=\"https:\/\/www.communitycare.co.uk\/2019\/05\/29\/service-users-left-isolated-councils-fail-meet-eligible-social-support-needs-say-social-workers\/\"><span class=\"s2\">With LAs already struggling to meet their legal duties to provide social care to vulnerable adults before the pandemic<\/span><\/a><\/span><span class=\"s3\">, and PM Boris Johnson <a href=\"https:\/\/www.independent.co.uk\/news\/uk\/politics\/boris-johnson-social-care-plan-bbc-breakfast-election-latest-a9282611.html\"><span class=\"s4\">admitting that the solution to the social care crisis is still five years away<\/span><\/a><\/span> <span class=\"s3\">it is unsurprising that LAs have <\/span><span class=\"s1\">increasingly become reliant on informal care. Even before COVID-19, <a href=\"http:\/\/www.smf.co.uk\/publications\/caring-for-carers\/\"><span class=\"s2\">7.6 million family carers in the UK were recorded to be providing around 149 million hours of care per week \u2013 equal to 4 million full-time paid carers<\/span><\/a>. It is likely that the number of informal care hours will have increased hugely amidst the pandemic, with social care services (e.g. day centres) closing and care staff self-isolating.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Last year, I interviewed 21 older carers (average age 75; 20% were 80+) who were caring for their family members with learning disabilities and\/or autism (see <a href=\"https:\/\/www.bath.ac.uk\/publications\/report-confronting-a-looming-crisis\/attachments\/Final_AckOnly.pdf\"><span class=\"s5\">\u2018Confronting a looming crisis\u2019<\/span><\/a><\/span><span class=\"s5\">). <\/span><span class=\"s1\">The accounts from carers demonstrated their huge capacity, energy and resilience to provide the best possible quality of life for their loved ones as they enter old age. Yet four main issues common to the group highlighted their struggles as they grow older and, inevitably, frailer \u2013 outlined here (identifiable characteristics have been altered):<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><b><i>Transition to retirement \u2013 a misnomer<\/i><\/b><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Carers reported having taken on more caring obligations, not fewer, as they entered retirement age, due mainly to the impact of health deterioration on themselves as well as of their loved ones. In general, people with learning disabilities and\/or autism experience a poorer health profile, particularly as they get older, compared to the general population. In the absence of specialist learning disability health care (e.g. only one of those interviewed mentioned they had access to a learning disability nurse), many reported that they themselves provided direct health as well as social care. Other behavioural changes linked to the ageing process could be very challenging for some family carers to manage: <\/span><\/p>\n<blockquote>\n<p class=\"p1\"><span class=\"s1\"><i>\u201cRoger has definitely become more autistic, more routines. I am having a real battle with him.\u201d <\/i><\/span><\/p>\n<\/blockquote>\n<p class=\"p1\"><span class=\"s1\">Their caring role had become extended and more complex. It now included: arranging transport for their loved one to get to and from GP and hospital appointments; administering medications; adjusting weekly timetables including liaising with support workers and other professionals; \u2018checking up\u2019 on support workers to make sure that tasks were carried out properly, or \u2018keeping an eye on\u2019 their loved one and arranging services. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Since no \u2018informal carer job description\u2019 exists, carers reported feeling as though they were taking roles that would ordinarily belong to formal paid carers and care managers. Such willingness to continue caring also risked being hidden from social services. In some cases, the onus was put on family carers (who are tired and have no time) by professionals to \u2018look up information\u2019 themselves using \u2018toolkits\u2019, and do tasks that the professional could do much more efficiently:<\/span><\/p>\n<blockquote>\n<p class=\"p1\"><span class=\"s1\">\u201cI<i>t feels like we have to know everyone else\u2019s job [care professionals] [but] they only have to know about us\u201d<\/i>. <\/span><\/p>\n<\/blockquote>\n<p class=\"p1\"><span class=\"s1\">There was virtually no respite from the day to day tasks of the family carers: <\/span><\/p>\n<blockquote>\n<p class=\"p1\"><span class=\"s1\"><i>\u201cWe have this thing that we are not entirely free\u201d (014)<\/i><\/span><\/p>\n<\/blockquote>\n<p class=\"p1\"><span class=\"s1\"><b><i>The Care Act \u2013 is it working?<\/i><\/b><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Whilst individuals talked positively about the Care Act 2014, the duty of the LA to undertake comprehensive needs assessments for both the person with a learning disability or their carer appeared not to be routinely or adequately performed:<\/span><\/p>\n<blockquote>\n<p class=\"p1\"><span class=\"s1\"><i>\u201cJoy has had a [needs] assessment and it really worries me and I can\u2019t say we noticed much difference once we had had it. They said \u2018you are not getting enough care but saying this, we can\u2019t afford it\u2019. The social worker said this - they would put her on a waiting list as a first step and if something comes available we would be notified\u201d (018)<\/i><\/span><\/p>\n<\/blockquote>\n<p class=\"p1\"><span class=\"s1\"><b><i>Challenges with services<\/i><\/b><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Issues included not having a consistent social worker or key person to deal with their family member\u2019s case, and perceived lack of expertise and pejorative attitudes of staff which threatened carer-social care trust:<\/span><\/p>\n<blockquote>\n<p class=\"p1\"><span class=\"s1\"><i> \u201c...umm social workers come and go \u2013 learning disability nurses come and go and of course life happens but nothing is standing still for my [loved one] and it makes for a very scary world for [them] I would say\u201d (005)<\/i><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><i>\u201d\u2026when I had complained about the lack of support we were getting, she said \u201cOh when did [loved one] catch Down\u2019s syndrome?\u201d <\/i>(007)<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"> \u201c<i>Social services have been patronising to me. I am capable and competent\u201d (016)<\/i><\/span><\/p>\n<\/blockquote>\n<p class=\"p1\"><span class=\"s1\"><b><i>Fears for the future<\/i><\/b><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Everyone talked about their fears for the future which mainly centred on what would happen to their disabled loved ones if they were to die. For most participants who had spent a lifetime \u2018managing\u2019 care as well as doing the practical tasks of caring, the question of who would take over their role was pressing:<\/span><\/p>\n<blockquote>\n<p class=\"p1\"><span class=\"s1\">\u201c<i>I hope [loved one] goes [dies] before me. We have organised [their] funeral\u2026I was ill earlier this year and so I am anxious..\u201d (014)<\/i><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><i>\u201cAs I get older\u2026there is nothing I can do \u2013 I do my best. All I want is to see [them] well treated. My aims are modest. I just want [them] to be safe, healthy and happy but [they are] none of those at the moment.\u201d (016)<\/i><\/span><\/p>\n<\/blockquote>\n<p class=\"p1\"><span class=\"s1\">That all of the study participants are now in the<span class=\"Apple-converted-space\">\u00a0 <\/span>COVID-19 \u2018vulnerable\u2019 category will only have heightened such fears. A recent follow up with some of the carers revealed other impacts of the pandemic. For example, day centre closure under lockdown, with no alternative services has reduced care hours by 100% for some individuals. Many have also experienced cuts to their domiciliary care \u2013 with little or no prior discussion from the local authority.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">As a consequence, one carer reported that their loved one \u201c<i>is now being left alone in their flat for 19 hours a day\u201d. <\/i>Another carer said that although their adult child appeared settled and \u2018placid\u2019, they worried that he was not eating well. Still other parents had been asked to host their family member with a learning disability and additional health issues during the pandemic; their supported living environment with staff changes now deemed to be too high risk. These issues reflect difficulties found across the country. <a href=\"https:\/\/www.challengingbehaviour.org.uk\"><span class=\"s2\">The Challenging Behaviour Foundation (CBF)<\/span><\/a>, a charity supporting families of people with learning disabilities, is recording unprecedented enquiries for help and support. Vivien Cooper OBE and CEO of CBF comments: <\/span><\/p>\n<blockquote>\n<p class=\"p6\"><span class=\"s1\"><i>\u201cFamilies are important to us all and often provide a lifetime of love, care and support for relatives with learning disabilities. This care and support can be significant, whether provided within the family home or when the person moves out of the family home.\u201d<\/i><\/span><\/p>\n<p class=\"p6\"><span class=\"s1\"><i>\u201cCurrently, with the additional pressures and concerns, acknowledgement of the crucial role families play and providing them with practical support should be a high priority nationally - but there is little evidence of that so far. Access to information and support can be difficult, especially if they have no internet access when Government and NHSE guidance is published only online.
